Sciatica describes pain radiating along the sciatic nerve, typically affecting one leg. Pain often shoots from the lower back through the buttock and down the leg, sometimes reaching the foot.

What Causes Compression

The sciatic nerve is the body's longest nerve, and several structures can compress it. A herniated lumbar disc is the most common cause. Piriformis syndrome—tightness in the deep buttock muscle—can also compress the nerve. Spinal stenosis narrows the canal through which the nerve travels.

Initial Management

Ice reduces inflammation in the acute phase (first 48-72 hours). Heat loosens tight muscles in the subacute phase. Gentle stretching of the piriformis and hamstrings relieves symptoms in many cases. The figure-four stretch and prone knee flexion are particularly effective.

Non-steroidal anti-inflammatory medications provide short-term relief while underlying inflammation resolves. Physical therapy addressing both the lower back and hip improves outcomes.

The Recovery Timeline

Most sciatica episodes resolve within 6-12 weeks with conservative treatment. Complete nerve irritation resolution may take several months, but pain typically improves much faster.

When Intervention Becomes Necessary

Surgery is considered when symptoms persist beyond 12 weeks despite conservative care, or when nerve damage progresses. However, research shows that most cases resolve without surgical intervention.

Prevention Strategies

Regular hip and glute strengthening prevents recurrence. Maintaining flexibility in the hips and spine reduces nerve compression risk. Proper sleeping position and ergonomic workspace setup support long-term relief.
